Dude fuck getting your car repaired at a dealership. Once you buy the car they couldn't give 2 fucks. Find yourself a good mechanic and build a rapport with them. They love repeat business and will give you a break to keep you coming back. Support the little guys. If people keep using the dealerships the little guys will eventually die off and you won't have a choice but to use the dealerships and get ripped off for substandard quality of service.

I only went to the dealer because its still under an extended warranty so I just have to pay a deductible whenever it needs to be repaired.
Believe me, I hate dealerships. I do the routine maintenance on it myself. If I didn't have a warranty I would take it to a local shop.

yeah, i'm totally fine. everything could be way worse. it's just some body damage and it still drives fine. it's just a fucking bummer. i do have insurance on it, so we'll see how it shakes out.

Can I ask what kind of car it is?
It was probably still making engine noise because of the coolant in the oil as it messes with the viscosity and is not a proper lubricant which can lead to bearing noise (but rarely any issues that require tearing the motor apart like that), so they had no reason to tear apart the bottom end like that. If they've all ready replaced the head gasket theres almost no other places where it could leak in to the motor like that. My first thought after the head gasket would be the radiator if it has a built in oil cooler, as the only other junctions where oil and coolant would have the chance to mingle would be in the engine block its self which would make me believe that there is a crack somewhere in the engine between the oil and coolant passages. At which point it would have similar symptoms to a blown head gasket except that it wouldn't the gasket, BUT that is a very very rare case they usually comes from contant overheating of the engine, or long term wear an tear (say after 200k miles or so).
I can tell you one thing. I work at a place that offers extended warranties on the cars they sell, as well as servicing the cars with the warranties. To top it all off they actually own the warranty company too.... The techs that work on cars when they know it has a warranty and they are guaranteed to get paid will throw all kinds of repairs at it knowing they won't fix the problem so that they can milk a paycheck out of the car. It's shady but the automotive business is a dog eat dog world.

So when I originally brought it in, it was making noise and the first diagnosis was blown head gasket because of the coolant in the oil. Head was sent to a machine shop to be cleaned and inspected; everything turned out good; dealership put in a new head gasket. Still made noise. Late last week, fan started coming on when I was idling but I didn't think much of it. Noticed on Friday when it was -10 out, I warmed up my car for 10 minutes and got in to drive to work, no heat. Filled the reverse tank; check the oil but couldn't tell if it was creamy or not. Got to work, refilled the reverse (I think the radiator was totally out so the first fill didn't do the trick). Drove errands over the weekend checking the coolant periodically and seems to have leaved out until I checked before I went to work this morning; empty again; check oil and there was no doubt coolant was in there. No engine noise this time, at least nothing that sounded out of the ordinary.

received 2 new pedals and a pocket piano today. awesome, right? all three were non working. i cant even blame the sellers/senders because they were all from different people so im guessing they all were working but all my packages got used for soccer in the ups truck.

That was pretty much the point I was trying to make. Car dealerships are shiftier than shithouse rats.

I GAS for so many pedals, but don't want to trade/sell any of my own or spend money because I'm saving up for something else (grown up stuff, boo). I'm having a pedal shootout today. Losers will be sold/traded. Maybe.

Well I did some research and found a few things....
First:
Reports are surfacing on the internet of problems with head-bolts (not typically a part that wears-out) becoming stripped out of the aluminum block in some applications the 2AZ-FE engine, including but perhaps not limited to the Toyota RAV4 and Toyota Camry. The problem begins when coolant level begins to drop and on investigation a coolant leak is discovered on the intake side of the block where a piece of foam is installed, evidently to shield the plastic intake manifold from heat. Speculation is that a harmful heat situation develops in this area of the engine block where the head-bolts are only shallowly threaded. When the problem is investigated, it is typically reported that three head-bolts located at this area of the engine may be pulled-out with very little effort, practically by hand. Since the threads in the block are stripped, the head-bolts cannot be simply tightened. Left unimpaired, catastrophic engine failure may result. This apparent design flaw is a very costly repair that may appear after the warranty has expired. Adding to the gravity of the problem, owners are reporting that Toyota is not offering a solution other than the vehicle owner paying Toyota dealers $3000.00 (USD) or more to replace the engine block, or "very limited" assistance, especially if the owner has chose to perform scheduled maintenance on the vehicle his self or herself rather than paying a Toyota Dealer's Service Department to do so resulting in that owner's lack of loyalty being taken into consideration. The exact range of specific engines and models that may develop this problem is not apparent without more information being released, and that may become available as more and more people report problems. A cottage industry is sprouting with repair kits being sold for over $300.00 (USD) to tap-out and install threaded inserts. This is reportedly a three-day repair that requires installing additional gaskets and/or parts at further cost.
The other reports are the dealers finding hairline cracks in the head between the oil and coolant passages. but IMO there is no way a hair line crack would allow enough coolant to get into the motor for it to be empty over night or even within a couple days. Your in luck if you don't have bearing noise but you need to go back armed with the information above and get them to fix it correctly and give you a warranty on the bearings they just installed because if coolant got in there again they could be compromised yet again.

Thanks for what seems to be a more put together bit of information than I found of people on forums discussing. I heard about the head-bolt issue after googling the problem after I first brought the car in during mid December. Pointed that out to the service manager and he said he had never seen/heard about it before (at least on cars that came in). I will definitely be pointing that out to him yet again, when he calls to give me an update on what the warranty wants to do.
Bearing noise was definitely before the first round of repairs and wasn't apparent after they replaced them. As you recommended I will ask them for a warranty on the bearings if they don't end up replacing them again.

No problem! I would definitely point it out again, its their job to try and keep costs of warranty repairs to a minimum plus the warranty companies usually setup a repair protocol for known issues that could turn into recalls. Don't let them win, and if they start getting lippy ask them for their customer service and corporate complaint phone numbers. That usually turns their attitude around quick.
Well if there's no bearing noise at the moment you should be good but the longer those bearings sit in that mix of oil and coolant the more likely they are to start having issues if not now but in the not so far future.
